Who Will Get It ?
Phimosis only occur in uncircumcised males and is more common in boys than men.
Normal in uncircumcised babies and toddlers, as the foreskin is attached to the glans. It will start detach naturally between 2-6 years old.
Older boys: It might occur in older boys and adults with:
- Repeated urinary tract infections
- Foreskin trauma
- Foreskin infection
- Repeated rough handling of the foreskin
Skin condition/dermatitis: ie eczema, psoariasis, lichen planus, lichen sclerosus
- These condition leads to inflammation and injury to the skin and form thickening of skin or scars and eventually lead to phimosis.
Risk factors for Phimosis:
- STD
- Diabetes Mellitus
- Recurrent UTI
What are the Symptoms of Phimosis ?
- Redness, soreness or swelling
- Affect incomplete bladder emptying
- Inflammation of penis called balanitis – soreness, itchiness and odor, redness and swelling, a buildup of thick fluid, pain when urinating
- During sex, phimosis may cause pain, cracked skin or lack of sensation
What are the Treatments Options for Phimosis ?
The treatment options for phimosis are determined by the symptoms. The majority of balanitis cases can be easily treated with good hygiene, creams, and ointments.
To improve hygiene, people are advised to clean their penis daily with lukewarm water and gently dry it. They should avoid using soap, bubble bath, or shampoo on their genitals and should urinate under the foreskin.
If the balanoposthitis was caused by a fungal or bacterial infection, an antifungal cream or antibiotics may be required.
Doctors may recommend treating the phimosis itself in cases of severe or recurring balanitis or balanoposthitis. They may recommend steroid creams to soften the foreskin.
Non-surgical treatment options might be just temporarily measure. The following step would be surgery, circumcision.
Circumcision would be ideal if:
- struggling to pull the foreskin
- too much scar tissue over the foreskin
- failure of medical treatment (cream)
- balanitis xerotic obliterans (BXO) is causing the phimosis and steroid creams are ineffective,
- Sexual activity uncomfortable.
- Urinary tract problems and is linked to an increased risk of penis cancer.
What is Retracted Foreskin ( Paraphimosis ) ?
Retracted foreskin is a medical condition that affects uncircumcised men in which the foreskin is retracted, either intentionally or unintentionally, and cannot be returned to its normal position.
If you suspect you have paraphimosis, you should see your doctor right away.

After reducing the swelling with a cold pack, steroid creams, or both, experienced medical practitioners may be able to manually pull the foreskin back. He/she may need to induce a few holes in the swollen foreskin with sterile needles.
If this is not successful, you will need to be admitted to the hospital for circumcision under general anaesthesia.
As previously stated, failure of non-operative methods to reduce paraphimosis is an indication for surgical intervention, in this case an emergency circumcision.
